Lunch Monitoring Form
Purpose
This form provides a structured record of every puppy feeding session at daycare. It ensures that food and water intake is monitored, any concerns are identified early, and a complete audit trail is maintained for each dog's nutritional welfare. Completion of this form is mandatory for every feed session.
1. Instructions
- Complete one row per puppy for each feed session
- Record the actual time food is placed in the crate and the time bowls are removed
- Note the percentage of food consumed (e.g., "All", "3/4", "1/2", "Refused")
- Record water intake as "Normal", "Low", or "None"
- Use the observations column to note any concerns: refusal, vomiting, guarding, lethargy, excessive speed, loose stools
- The supervising staff member must sign each session
- At the end of each feed, photograph the completed form and upload to the Docs WhatsApp group

3. Escalation Triggers
The following observations must be reported to management immediately — do not wait until the end of the day:
Immediate Escalation Required
- Complete food refusal for two consecutive sessions
- Vomiting during or immediately after feeding
- Diarrhoea or blood in stools
- Choking or difficulty swallowing
- Aggressive guarding behaviour that poses a risk to staff
- Any puppy showing signs of lethargy, pain, or distress
4. Record Retention
Completed Lunch Monitoring Forms are retained for a minimum of 12 months. They form part of the welfare documentation that may be reviewed by the licensing authority during inspections.
